高容量経口ビタミンDで転倒と骨折が増加したという論文を紹介します。この論文の存在は、名郷直樹先生に教えていただきました。どうもありがとうございます。

この論文は全文見ることができるようです。
1年に1回秋から冬に50万IUのビタミンDもしくはプラセボを地域在住の高齢女性に投与して、転倒と骨折を評価した大規模なランダム化比較試験です。
結果としてビタミンD投与群では転倒のリスク(1.15; 95%CI, 1.02-1.30)と骨折のリスク(1.26; 95% CI, 1.00-1.59)が有意に増加しました。特に投与後最初の3ヶ月間で、その後の9ヶ月間より転倒が多いという結果でした。
ビタミンDは転倒予防に有効というメタ分析があるのに(有効でないというメタ分析もありますが)、1年に1回の経口投与ではむしろ転倒が増えるという逆の結果です。少なくとも高容量ビタミンDの単回経口投与は行うべきではないといえます。ビタミンDの研究論文は少なからずありますが、結果が一定しないのが本当に困ります。これが現実かもしれませんが。
Abstract
CONTEXT: Improving vitamin D status may be an important modifiable risk factor to reduce falls and fractures; however, adherence to daily supplementation is typically poor.
OBJECTIVE: To determine whether a single annual dose of 500,000 IU of cholecalciferol administered orally to older women in autumn or winter would improve adherence and reduce the risk of falls and fracture.
DESIGN, SETTING, AND PARTICIPANTS: A double-blind, placebo-controlled trial of 2256 community-dwelling women, aged 70 years or older, considered to be at high risk of fracture were recruited from June 2003 to June 2005 and were randomly assigned to receive cholecalciferol or placebo each autumn to winter for 3 to 5 years. The study concluded in 2008.
INTERVENTION: 500,000 IU of cholecalciferol or placebo.
MAIN OUTCOME MEASURES: Falls and fractures were ascertained using monthly calendars; details were confirmed by telephone interview. Fractures were radiologically confirmed. In a substudy, 137 randomly selected participants underwent serial blood sampling for 25-hydroxycholecalciferol and parathyroid hormone levels.
RESULTS: Women in the cholecalciferol (vitamin D) group had 171 fractures vs 135 in the placebo group; 837 women in the vitamin D group fell 2892 times (rate, 83.4 per 100 person-years) while 769 women in the placebo group fell 2512 times (rate, 72.7 per 100 person-years; incidence rate ratio [RR], 1.15; 95% confidence interval [CI], 1.02-1.30; P = .03). The incidence RR for fracture in the vitamin D group was 1.26 (95% CI, 1.00-1.59; P = .047) vs the placebo group (rates per 100 person-years, 4.9 vitamin D vs 3.9 placebo). A temporal pattern was observed in a post hoc analysis of falls. The incidence RR of falling in the vitamin D group vs the placebo group was 1.31 in the first 3 months after dosing and 1.13 during the following 9 months (test for homogeneity; P = .02). In the substudy, the median baseline serum 25-hydroxycholecalciferol was 49 nmol/L. Less than 3% of the substudy participants had 25-hydroxycholecalciferol levels lower than 25 nmol/L. In the vitamin D group, 25-hydroxycholecalciferol levels increased at 1 month after dosing to approximately 120 nmol/L, were approximately 90 nmol/L at 3 months, and remained higher than the placebo group 12 months after dosing.
CONCLUSION: Among older community-dwelling women, annual oral administration of high-dose cholecalciferol resulted in an increased risk of falls and fractures.

結果としては、筋肉、バランス、歩行のいずれも有効という報告と無効という報告があり、現時点では結論を出せる状況ではないとのことです。今回はメタ分析は実施されていません。
転倒予防に関してはビタミンDが有効な可能性がありますが(コクランの系統的レビューでは無効とされていますが、その他の系統的レビューでは有効とされています)、身体機能の改善に関してはまだどちらともいえないというところです。
そのため、身体機能改善を目的としたビタミンDの使用は、ビタミンDが欠乏している患者に限定されると思います。ビタミンDが不足している患者にルーチンに使用するほどのエビデンスはないと考えます。
Abstract
Objective: The objective of this study was to systematically review all the published articles examining the effects of low serum vitamin D concentration and vitamin D supplementation on muscle, balance and gait performance among people aged 65 and older.
Methods: An English and French Medline search ranging from January 2004 to November 2008 indexed under the Medical Subject Heading (MeSH) terms "aged OR aged, 80 and over" AND "Vitamin D OR Vitamin D Deficiency" combined with the terms "Gait" OR "Gait Apraxia" OR "Gait Disorders, Neurologic" OR "Walking" OR "Mobility Limitation" OR "Polyneuropathy" OR "Proprioception" OR "Ataxia" OR "Accidental Falls" was performed.
Results: Of the 102 selected studies, 16 met the selection criteria and were included in the final analysis. There were 8 observational studies and 8 interventional studies. The number of participants ranged from 24 to 33067. A majority of studies examined community-dwelling older women. Five observational studies showed a significant positive association, whereas three studies did not. Four of the 5 studies and two of the 3 studies which tested the vitamin D supplementation effect, respectively on balance and gait, showed no significant effect. Four studies showed a significant effect on muscle strength, while this effect was not observed in three others studies. In addition, there was no significant association between vitamin D supplementation and an improvement of the sit-to-stand test results in 50% of the studies.
Conclusions: The findings show that the association between vitamin D and physical performance remains controversial. Observational studies and clinical trials yielded divergent results, which highlights the complex and to date still poorly understood association between serum vitamin D concentration or vitamin D supplementation and physical performance.

高齢者の転倒予防は運動とビタミンD
米国予防医療専門委員会(USPSTF)による、高齢者の転倒予防に対するプライマリ・ケアにおける介入の論文を紹介します。

今回はプライマリ・ケアにおける地域在住の高齢者の転倒予防に関するエビデンスの紹介をしています。一定以上の質のあるRCTのみをまとめた報告ですので、エビデンスレベルはかなり高いものになります。
結論として運動・理学療法とビタミンDに関しては、それぞれ転倒予防に有効としています。多方面からの介入(interventions with comprehensive management)は統計学的有意差は認めませんでしたが有効な傾向にあります。
これより転倒リスクがある高齢者には転倒予防として、運動・理学療法の指導とビタミンDの投与を行うべきといえます。高齢者にルーチンにビタミンDを投与するのは副作用も含めてどうなのだろうという思いは個人的にありますが、転倒予防に関するメリットは明らかです。
特にビタミンDが不足~欠乏している高齢者(25ハイドロキシビタミンDの血中濃度が30ng/ml以下)では投与すべきということになります。ただ、日本人の平均値で男性28.5g/ml、女性24.2ng/dlという論文がありますので、これを参考にすると大半の高齢者はビタミンD不足です。
ちなみに日本で医療保険で測定できる1,25ハイドロキシビタミンDでは、ビタミンDが本当に不足~欠乏しているかどうかの判断はできないというエビデンスがありますので、25ハイドロキシビタミンDの血中濃度を調べることが必要です。かなり高価ですが…。

BACKGROUND: Falls among older adults are both prevalent and preventable.
PURPOSE: To describe the benefits and harms of interventions that could be used by primary care practitioners to prevent falling among community-dwelling older adults.
DATA SOURCES: The reviewers evaluated trials from a good-quality systematic review published in 2003 and searched MEDLINE, the Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, and CINAHL from the end of that review's search date to February 2010 to identify additional English-language trials. STUDY SELECTion: Two reviewers independently screened 3423 abstracts and 638 articles to identify randomized, controlled trials (RCTs) of primary care-relevant interventions among community-dwelling older adults that reported falls or fallers as an outcome. Trials were independently critically appraised to include only good- or fair-quality trials; discrepancies were resolved by a third reviewer.
DATA EXTRACTION: One reviewer abstracted data from 61 articles into standardized evidence tables that were verified by a second reviewer.
DATA SYNTHESIS: Overall, the included evidence was of fair quality. In 16 RCTs evaluating exercise or physical therapy, interventions reduced falling (risk ratio, 0.87 [95% CI, 0.81 to 0.94]). In 9 RCTs of vitamin D supplementation, interventions reduced falling (risk ratio, 0.83 [CI, 0.77 to 0.89]). In 19 trials involving multifactorial assessment and management, interventions with comprehensive management seemed to reduce falling, although overall pooled estimates were not statistically significant (risk ratio, 0.94 [CI, 0.87 to 1.02]). Limited evidence suggested that serious clinical harms were no more common for older adults in intervention groups than for those in control groups. Limitations: Interventions and methods of fall ascertainment were heterogeneous. Data on potential harms of interventions were scant and often not reported.
CONCLUSION: Primary care-relevant interventions exist that can reduce falling among community-dwelling older adults.

がん患者における栄養スクリーニングと低栄養の早期治療

がん患者における栄養スクリーニングと低栄養の早期治療に関するレビュー論文を紹介します。

詳細な栄養評価には以下の5つの項目が必要とあります。詳細な栄養評価では当然、どれも重要です。ただ、単独で最も重要なパラメーターは体重減少とありました。
1. Social demography (gender, age, professional status, and living conditions)
2. Primary tumor (site, disease stage at diagnosis) and the management of the disease (previous and on-going treatments)
3. Anthropometry (actual weight, body mass index [BMI, the weight in kilograms divided by the height in centimeters squared], usual healthy body weight, unintentional weight loss since the start of the illness, or appearance of the first symptoms; weight loss during the last week, the last month, and over the last 6 months;
4. Clinical examination, Subjective Global Assessment and Karnofsky Index (evaluating functional capacity)
5. Biochemical data: serum albumin, prealbumin, total lymphocyte count cholesterol, C reactive protein (CRP), pseudocholinesterase (PChE)
このうち、Pseudocholinesterase(偽性コリンエステラーゼ)は私は初めて聞きました。コリンエステラーゼは栄養指標に用いますが、偽性コリンエステラーゼとの違いはよくわかりません。ご存知の方がいましたら、教えていただけると嬉しいです。よろしくお願いいたします。
論文に掲載されていた栄養評価のアルゴリズムを示します。概ねこの通りに進めていけばよいかと思いますが、1点だけ納得いかないところがあります。消化管を使用できるかどうかの後に嚥下障害がないかどうかを判断していますが、この順番はおかしいと考えます。嚥下障害の有無を先に評価して、その上で消化管を使用の可否を判断すべきです。
がんのリハビリテーションの項目が論文内にあることは嬉しいです。栄養の論文の中にリハの記載が、リハの論文の中に栄養の記載がもっと増えることを期待しています。
Abstract
Background Malnutrition is a frequent complication in
patients with cancer and can negatively affect the outcome
of treatments. On the other hand, side effects of anticancer
therapies can also lead to inadequate nutrient intake and
subsequent malnutrition. The nutritional screening aims to
identify patients at risk of malnutrition for prompt treatment
and/or careful follow-up.
Methods and results This manuscript highlights the need of
an interdisciplinary approach (oncologist, nutritionist, dietitian,
psychologist, etc.) to empower patients who are
experiencing loss of physiological and biological function,
fatigue, malnutrition, psychological distress, etc., as a result
of cancer disease or its treatment, and maintain an
acceptable quality of life.
Conclusions It is necessary to make all healthcare professionals
aware of the opportunity to identify cancer patients
at risk of malnutrition early in order to plan the best
possible intervention and follow-up during cancer treatment
and progression.

スポーツ栄養マネジメント
鈴木志保子著、健康づくりと競技力向上のためのスポーツ栄養マネジメント、日本医療企画を紹介します。

スポーツ栄養の本は少なからずありますが、私が今まで読んだ中ではもっとも参考になる書籍でした。ちなみに鈴木志保子先生は日本スポーツ栄養研究会の会長です。以前、神奈川県立保健福祉大学に行ったときに、ご挨拶だけさせていただいたことがあります。
第2章にスポーツ栄養マネジメントの目的として以下の3つが紹介されています。
①リスクマネジメント
②身体活動が増加した状況での健康維持と疾病予防
③体力・競技力向上(健康の増進を含む)
これはリハ栄養マネジメントの目的にも応用することができます。以下のような感じでしょうか。
①低栄養や不適切な栄養管理下におけるリスクマネジメント
②機能訓練の時間と内容が増加した状況での適切な栄養管理
③筋力・筋肉量・体力向上
今までのリハ栄養では主に①について述べてきた気がします。今後は②、③についても述べていきたいと思います。栄養状態と栄養管理を考慮したリハ・機能訓練だけでなく、リハ・機能訓練を考慮した栄養管理にも重きを置きたいと考えます。
第6章の省エネな身体、超省エネな身体の考え方も参考になりました。基礎代謝を極力少なくすることで、少ないエネルギー摂取量で長時間の運動・スポーツに耐えうる身体になっているのですね。健康的ではありませんが、スポーツ選手のパフォーマンスを最大限発揮するという意味では、目的に適っていると感じます。リハ栄養では省エネな身体は不要ですが…。
リハ栄養に参考になる知見が多いだけでなく、スポーツ栄養の豊富な経験から得られた知見がまとまっており、スポーツ栄養の学習にもリハ栄養の学習にもなると思います。特に管理栄養士にご一読をお勧めします。
