第1回神奈川心不全栄養研究会
7月3日(木)19時30分からTKPガーデンシティ横浜で第1回神奈川心不全栄養研究会が開催されます。詳細は上記の案内ビラと参加申込用紙をご参照ください。

書籍「悪液質とサルコペニア」上梓
本日、荒金英樹、若林秀隆編著、悪液質とサルコペニア‐リハビリテーション栄養アプローチ、医歯薬出版が上梓されました。

7th cachexia conference参加
12月9-11日と神戸で7th cachexia conferenceが開催されました。私は3日間フル参加してきました。

私の見立てなのでかなり違うかもしれませんが、参加者は400(~500)人程度で、日本人の参加者は100人もいなかったのではと思います。世界中のサルコペニアと悪液質の一流の研究者が日本に集まっていたのに、とてももったいないことだと感じました。
サルコペニアへの関心はこの数年で高まってきましたが、悪液質の新しい考え方への関心はまったく高まっていないことがよくわかりました。日本でも悪液質への関心を高めるよう、努力しなければいけないと痛感しました。学会の宣伝不足の要素もありそうですが・・・。
参加者の多くは研究家であって、臨床家ではなさそうでした。臨床家ではリハ、循環器、緩和医療、呼吸器の医師がそれぞれ数人ずつ参加していたようです。リハ医は私を含めて6人でした。それだけ研究と発表のレベルは高く、日本が遅れていることを実感しました。
リハ栄養的に一番印象に残ったのは、悪液質の薬物療法の研究が進んでいますが、栄養、運動、心理社会サポートをしっかり行うことの重要性が繰り返し述べられていました。前悪液質・悪液質の早期発見と包括的対応といったリハ栄養を、臨床現場で実践できるよう還元したいです。
私はAssociation between muscle strength of head lifting, dysphagia, and malnutrition in Japanese elderly: possibility of sarcopenic dysphagiaという発表をしてきました。
口腔・嚥下に関する発表は、私と熊本リハ病院の吉村先生のAssessment of oral cavity function and its clinical significance: Is ROAG valuable for sarcopenia screening?だけでした。
つまり、口腔・嚥下のサルコペニア・悪液質に関しては未開の領域であり、ここでは日本が先行できる可能性があります。また、サルコペニア肥満の発表もほとんどありませんでした。ここは韓国が頑張っている領域ですが、日本も頑張れそうです。
アジアでのサルコペニアのコンセンサス論文が近日中に発表になると言っていました。重要な論文になりますので、納得できる内容の論文であれば日本で広めたいですし、そうでなければレター論文を書いてコメントしたいと思っています。できるかわかりませんが(笑)。
吉村先生に撮影していただいたポスター発表の様子の写真を掲載いたします。英語での発表やディスカッションをもっとできるようにしなければと思いつつ、なかなか改善できませんね(苦笑)。

悪液質:免疫系・脳・代謝の相互作用

炎症性疾患という切り口ですので、内容的にはサイトカインに関する記載が多いです。悪液質の定義についてはEvansらの定義を用いています。しかし、Fearonらの定義は引用されていませんので、2010年までの文献をレビューした書籍だと思います。
TNF-α、IL-1、IL-6といった悪液質に関するサイトカインが上昇する疾患として、がん、COPD、エイズ、心不全、腎不全、関節リウマチ、敗血症が挙げられています。敗血症というと急性疾患をイメージしがちですが、慢性的に敗血症が持続する場合には悪液質と考えてよいようです。
結論として、薬物療法で単一のサイトカイン(例えばTNF-α)を抑制するだけでは、悪液質は予防できないとあります。今後は複数のサイトカインを抑制する薬物療法に関する研究が必要とのことです。
ちなみに栄養療法や運動療法に関する記載はありませんでした。これらによる抗炎症作用に関する記載が少しはあってもよいのに、と思うのは私だけでしょうか。

myopeniaの概念確認
PEN(静脈経腸栄養ニューズ)の最新号(338、2012年1月1日号)に、myopeniaに関する記載がp3にありました。ちょっと気になりましたので、その部分を引用します。以下、引用です。
「骨格筋量減少は臨床上二つの病態によくみられる。一つは加齢に伴うもので、いま一つは癌である。サルコペニアはむしろ加齢に伴う骨格筋減少を表現するので、癌悪液質にみられるものに対しては新しい表現、すなわちmyopeniaと表現することを強く主張している」こと等を紹介している。
私はミオペニアについて「すべての原因による筋肉量と筋力の低下」と言ってきました。もちろん年齢は問いません。つまり、広義のサルコペニアと同義です。ミオペニアの論文では、以下のように提言されています。
We suggest the term “myopenia” to indicate the presence of clinically relevant muscle wasting due to any illness and at any age
しかし、上記引用ですと、加齢によるサルコペニアはミオペニアに含まれないような印象があります。そこでmyopeniaの概念を確認してみました。

図を見ても明らかなように、ミオペニアには加齢によるサルコペニアも含まれます。つまり、引用の意図は、サルコペニアという用語は狭義(加齢が原因の場合のみ)で使用して、ミオペニアという用語を癌悪液質が原因の場合だけでなくすべての原因、すべての年齢で使用しようということだと考えます。
ちなみにPENの同号のp3には、昨年の日本外科代謝栄養学会で私が発表したPrehabilitationも紹介されていました。また、サルコペニアについて割と多く紹介されています。PENを入手できる方はよかったら見てください。よろしくお願いいたします。

日本緩和医療学会ニューズレター:緩和ケアと栄養
日本緩和医療学会ニューズレター第53号に「緩和ケアと栄養」の教育講演に関する報告がされています。演者は東口先生で座長・報告は岡田先生です。

「実は終末期になると栄養障害から生じる免疫能の低下によって、がん自体ではなく感染症が直接的な死因となることが多いということが問題であると示された。そしてNST による適正な栄養管理が実践されると終末期がん患者の肺炎をはじめとする種々の感染や多発する褥瘡などの他疾患の合併が減少することも示された。間接熱量計を用いて終末期がん患者の代謝動態を検討した結果、臨床的な悪液質の出現時期とほぼ一致してエネルギー消費量が減少する。この時期がギアチェンジを全面的に実施すべき時期であり、栄養管理法もこの時点から変更すればいいということになる。臨床的な悪液質として「がん進展による高度全身衰弱あるいはコントロール不能な腹水、胸水、全身浮腫の発生」として示された。」
以上、引用です。確かに感染症が死因となることは少なくないと思います。ここでの「臨床的な悪液質」というのはFearonらの「不応性悪液質」に近いと私は解釈しています。Fearonらの「前悪液質」と「悪液質」の時期には、適正な栄養管理を実践することが重要で、「不応性悪液質」になったらギアチェンジすべきと考えます。
リハ栄養の立場でも、「前悪液質」と「悪液質」の時期には積極的な栄養管理が大切です。運動(有酸素運動の範囲内の歩行やレジスタンストレーニング)による悪液質への抗炎症作用を期待して運動指導しても、終末期のような1日200~600kcal程度の栄養管理では、運動は実施困難です。この鑑別診断が重要になりますね。

クローズアップ「癌悪液質の謎に挑む」
NCIキャンサーブレティンの2011年11月01日号のHPで、クローズアップ「癌悪液質の謎に挑む」が取り上げられています。悪液質の研究の現状について、日本語としてはよくまとまっている資料だと思います。

「悪液質は癌に限ったことではない。AIDS患者や、慢性の腎臓病、心疾患などのほか、重度の外傷または熱傷でも普通に見られる」
重度の外傷や熱傷の場合、慢性的に炎症が持続していればという限定付きだと思います。ただ、がん以外の様々な慢性炎症性疾患や慢性臓器不全でも悪液質を生じることを知らない医療人が少なくないのが現状です。

「もっとも研究が進んでいると思われるのが、テネシー州メンフィスに本社があるGTx社が開発した選択的アンドロゲン受容体調節剤のGTx-024(商品名:オスタリン)である。同社は8月にこの試験薬に関する第3相臨床試験を2つ立ち上げ」
「ミオスタチンの本来的な機能は筋肉成長のブレーキ役となることである。ミオスタチンの産生を支配する遺伝子に変異がある羊、マウス、犬、牛では過度に筋肉質となる。(中略)ミオスタチンおよびアクチビンの活性を阻害すると、悪液質に大きな効果をもたらす可能性がある。(中略)ミオスタチンおよびアクチビンを標的とすることで悪液質による心筋の喪失が改善される」
選択的アンドロゲン受容体調節剤やミオスタチン拮抗薬は、悪液質に対する薬物療法として、今後期待されます。悪液質では骨格筋が減少し広義のサルコペニアとなりますが、心筋のサルコペニアも生じることがあります。心筋の喪失が改善されるというのは興味深いです。
Lancet Oncologyに今年5月に掲載された癌悪液質の定義及び分類に関する国際的コンセンサスに関しては、以下のように訳されていました。以下もHPからの引用です。
「癌悪液質の診断基準
・過去6カ月以上にわたって5%を超える体重減少がある
・BMIが20未満で2%を超える体重減少がある
・補正四肢筋量が別の消耗性症候群であるサルコペニア(骨格筋減少症)と同等で、2%を超える体重減少がある
癌悪液質の病期分類
・前悪液質:体重減少は5%未満で、耐糖能異常や食欲不振を伴う
・悪液質:体重減少が5%を超え、他の症状や状態が悪液質の診断基準に一致する

refractory cachexiaをどう訳せばよいのか悩んでいたのですが、今回「不応性悪液質」と訳されていたので、今後は少し気楽にこの言葉を使えます。
あとはがん以外の疾患による悪液質でもこの定義と分類を使用してよいという報告やコンセンサスが早くでると、もっと嬉しいですね。私はがん以外の疾患でも使用してよいと勝手に解釈していますが、がん以外の場合、Evansの2008年の悪液質の定義と診断基準を使うのが本来は適切かもしれません。

anamorelinのグローバル第III相臨床プログラム開始
1か月前ほどの記事ですが、非小細胞肺がんに伴う食欲不振/悪液質患者を対象としたanamorelinのグローバル第III相臨床プログラムが開始された、というプレスリリースの記事がありました。

Anamorelinは経口投与可能なグレリン受容体作動薬です。本剤はこれまでに約500名の被験者への投与経験があり、361名のがん患者を対象とした4つの第II相試験が終了しています。第II相試験の最終結果は近く公表の予定です。
グレリンは主に胃で産生されるホルモンです。グレリンは「空腹」に関わるホルモンとして最初に特定され、その投与により、急速に食欲が刺激され、食物摂取量や体重の増加、また他にも除脂肪体重の増加や胃内容物排出の促進など生理作用の増強が引き起こされる可能性があります。

リハ栄養やサルコペニアで一番苦慮するのは、侵襲と悪液質のときのリハ栄養管理です。薬物治療も含めた包括的治療というのが現状ですが、Anamorelinが有効で臨床で使用できるようになれば、リハ栄養の包括的治療の幅が広がると期待しています。

アピタル‐ひょっとして認知症
アピタル‐ひょっとして認知症、というブログがあります。認知症学会の専門医、笠間睦先生が執筆されているものですが、第163回に「悪液質」に対する治療の試み[11/08/19]が掲載されています。

6th Cachexia Conferenceのプログラム
6th Cachexia Conferenceが12月8-10日にミラノで開催されます。

本来は日本で開催予定だったのですが、東日本大震災の影響でミラノに変更になりました。その代わり第7回Cachexia Conferenceは2年後の12月9-11日に日本で開催予定です。今回は参加できませんが、2年後はぜひ参加したいと思っています。

東日本大震災の影響のようです。その代わり、2013年12月9-11日に7th Cachexia Conferenceが日本で開催されるとのことです。日本での開催なので演題も出そうと思っていたのですが残念です。やむを得ないのかもしれませんが…。

癌と臨床栄養
今日は丸山道生編著、癌と臨床栄養、日本医事新報社を紹介します。 今月発刊となった新しい書籍です。

確かに今まで癌に関する基礎から臨床に至る代謝栄養をまとめた書籍はありませんでしたので、初めての教科書といえます。執筆陣も現場でNSTに積極的に取り組んでいる方が多く、実践的な内容となっています。
「癌患者は癌で看取る。栄養不良状態やトラブルでは看取らない」を合言葉にしているそうですが、これには共感できます。また、丸山先生が編集しているだけに在宅での癌の栄養管理やシームレスな地域連携に関する執筆は充実しています。
悪液質に関しては、このブログで紹介しているような悪液質、前悪液質に関する診断基準は掲載されていませんでしたが、癌の栄養管理を体系的に学習したい方にお勧めします。

悪液質に対する運動と薬物療法
今日は、5th Cachexia Conferenceの抄録集の中から、Exercise, anticatabolic strategies and the treatment of cachexiaの抄録を紹介します。
Exercise, anticatabolic strategies and the treatment of cachexia

悪液質ではないがんのリハにおける運動療法(積極的な有酸素運動とレジスタンストレーニング)の有効性はかなり検証されてきています。がんのリハはリハ領域でもトピックといえます。
ただ、悪液質のがん患者に対して、どの程度の運動療法を行うのがよいのかのエビデンスはまだ不明です。この抄録では、廃用を予防し身体活動を推奨する程度の運動は、多領域からの悪液質に対する包括的治療として記載されています。今後の研究課題ですが、現時点では維持的なリハは行うべきと考えています。
CRPがいくつ以下なら積極的リハ可能、いくつ以上なら維持的リハのみ可能という単純な基準があれば一番わかりやすいのですが、そのような基準も今のところはありません。ただ、プレアルブミンに関しては16以上なら積極的リハ可能、5以下なら維持的リハのみ可能とはいえると思います。
あと薬物療法として、メドロキシプロゲステロン、EPA(エイコサペンタエン酸)、L-カルニチン、サリドマイド(日本では奇形児の問題が大きかったので、現在は多発性骨髄腫でしか使えませんね)およびこれら4種類すべての5群間でがん悪液質への効果があるかをみたランダム化比較試験では、4種類すべて投与した群で除脂肪体重が有意に増加し疲労が改善し、大きな副作用は認めなかったようです。
日本でそのまま臨床応用できる研究結果では全くありませんが、悪液質に対しては原疾患に対する治療とともに、食事療法、運動療法、薬物療法の併用が必要なことは確かだと思います。NSTでもここまで踏み込んだ提案をすることが望ましいと考えています。
Physical activity, and even more so, exercise tolerance are greatly reduced (by approximately 40%) in cancer cachexia due to wasting of lean body mass and fatigue. On the other hand, as physical activity is also an important component of the physical and social domains of quality of life (QL), any restoration of physical activity levels and exercise training may translate into an improvement in QL for cachectic cancer patients [1].
Physical activity and exercise should require a normal muscle mass, and an adequate level of QL and motivation, both lacking in cachectic patients. The cachectic status itself and the related lack of physical activity, in turn, may worsen cachexia thus triggering a vicious circle. Therefore, it is crucial to act on both components, i.e. lean body mass and patient motivation. As for the increase of lean body mass a central role is played by anticatabolic strategies which should have as a target:
- the reduction of the production/release of the proinflammatory cytokines
- the inhibition of the muscle protein catabolism by inhibiting the ubiquitin/proteasome pathway(bortezomib and EPA)
- an increase on muscle protein synthesis (steroid androgen receptor modulators – SARMs -, oxandrolone)
- the increase of energy intake, by ghrelin and ghrelin mimetics
As regards the patient motivation, physical activity itself and exercise, such as programs of resistance training and aerobic exercise, may have an important role and may contribute, in turn, to an increase of lean
body mass [2]. When devising a therapeutic program for cancer cachexia, the emphasis should be placed on multimodal therapies that tackle all issues simultaneously. These interventions should combine highprotein nutritional supplements, the downregulation of proinflammatory cytokines and chronic inflammation by anti-inflammatory agents and routine mobilisation programs to prevent deconditioning and encourage physical activity. In this context, we have carried out a phase III randomised clinical trial in cachectic cancer patients to establish which was the most effective and safest treatment to improve lean body mass and fatigue as primary end-point, and as secondary endpoint, among others, the increase of physical activity level measured by a portable electronic device. Three hundred and thirty-two assessable patients with cancer cachexia were randomly assigned to one of five arms of treatment: 1 - medroxyprogesterone 500 mg/d or megestrol acetate 320 mg/d; 2 - oral supplementation with EPA; 3 - L-carnitine 4 g/d; 4 - thalidomide 200 mg/d; 5 - a combination of the above agents. Treatment duration was 4 months. Analysis of changes from baseline showed that LBM significantly increased in the combination regimen (arm 5).
Fatigue improved significantly in arm 5. Total daily physical activity showed that total energy , and active energy, expenditure increased significantly in arm 5. Toxicity was substantially negligible and comparable
between treatment arms [3].
The results of our trial, showing the efficacy of a combined treatment approach, seem to confirm the basic assumption that the treatment of cancer cachexia, a multifactorial syndrome, is more likely to yield success
with a multitargeted approach which may have the potential to reverse the predominant features of cachexia, i.e. progressive loss of muscle mass and function and improve the associated symptoms that affect QL.

悪液質ではないがんのリハにおける運動療法(積極的な有酸素運動とレジスタンストレーニング)の有効性はかなり検証されてきています。がんのリハはリハ領域でもトピックといえます。
ただ、悪液質のがん患者に対して、どの程度の運動療法を行うのがよいのかのエビデンスはまだ不明です。この抄録では、廃用を予防し身体活動を推奨する程度の運動は、多領域からの悪液質に対する包括的治療として記載されています。今後の研究課題ですが、現時点では維持的なリハは行うべきと考えています。
CRPがいくつ以下なら積極的リハ可能、いくつ以上なら維持的リハのみ可能という単純な基準があれば一番わかりやすいのですが、そのような基準も今のところはありません。ただ、プレアルブミンに関しては16以上なら積極的リハ可能、5以下なら維持的リハのみ可能とはいえると思います。

Society on Cachexia and Wasting Disorders
サルコペニアや悪液質に関心のある人なら以前から知っていたかもしれませんが、私は今日はじめてSociety on Cachexia and Wasting Disordersの存在を知りました。今までESPENの資料ばかりフォローしていましたが、この学会の資料も要フォローです。
