瀬田先生からの報告

瀬田先生からの貴重な報告が日本リハビリテーション医学会の掲示板に掲載されています。ただ、これだと学会員の目にしか触れませんので、こちらのブログで転載させていただくことにしました。以下、転載です。

一昨日までは、東北大学でも医療はDMATが機能していればよく、救出救命、水、食糧、電気等ライフラインの確保、遺体の収容の段階であると判断していました。
一昨日から昨日まで、避難所を中心の医療支援も必要と考え、東北大4名と神戸大4名で、避難所支援の観点から石巻に入り、状況確認をしてきました。
石巻においては、石巻日赤によるDMATがよく機能しており、救急搬送も含め来院された方へは、緊急時としては比較的良好な医療が提供(骨折は帰宅、肺炎も重症化していなければ帰宅)されておりましたが、避難所の健康管理には、ほとんど手が回っていない状況でした。
避難所を3か所視てきましたが、生徒300人程度の学校に1000人以上の避難民が集まっており、その半数以上が家を失った状況でした。1か所は日赤も把握していない状況でした。避難所の健康管理は、みてきた3か所すべて1~2名の養護教員によってされており、常用薬がなくなった方にどのように対応してよいか(どのくらいまで血圧が上がったら問題か?インスリンを注射できないのはそのままでよいか?ワーファリンが突然なくなってもいいのか?)、相談する相手もなく、その養護教員がもっている知識と経験で判断している状況でした。
避難生活は、これまでの震災の規模をはるかに上回る人数・期間になることが予想され、今は避難所の健康管理を支援するような、救護所が、できるだけ多く立ち上がることが急務と考えます。この任務は、医師であれば特別な専門性は必要なく、どんなバックグランドを持ったリハ医でもつとまると考えます。

また、もしリハ医としての専門性を発揮した支援を考えるのであれば、
各地域の拠点病院が復興しする1ヶ月後程度になると思いますが、救命された後寝かせきり、ADL障害のある避難民はフォーレが留置され寝かせきりという状況があると思いますので、それらの方の離床を支援できると思われます。今回の震災地域の大半がリハ科専門医が存在しない地域です。拠点病院に、臨時で短期間リハ医を派遣するなどして、離床支援をするという方法もあると思います。

数多くの摂食・嚥下障害の事例集で、臨床現場の知識がかなりわかりやすく執筆されています。また、摂食・嚥下障害患者のQOLの測定や、現場のチームワークを促進するコーチングが含まれている所が、出江先生のこだわりかと思います。

私も第2部事例編 6施設間・職種間連係の考え方 47)NST活動による栄養状態改善により劇的に改善した患者を執筆させていただきました。興味のある方はぜひご一読いただければと思います。

9月2-3日に出江先生が大会長で仙台で開催予定の、第17回日本摂食・嚥下リハビリテーション学会学術大会が無事開催されるのかどうか心配です。まだ先の話ですので、その前にやるべきことがたくさんありますが。

Exercise Physiology: Theory and Application to Fitness and Performance

Scott K. Powers, Edward T. Howley(著), Exercise Physiology: Theory and Application to Fitness and Performance 7th edition, McGraw Hill Higher Educationを紹介します。アマゾンでは5600円程度でペーパーバックを購入できます。ただ、今年の11月に8th editionが出版されるようですので、それまで購入を待ってもよいかもしれません。

英語のリハ医学の代表的な教科書であるPhysical Medicine and RehabilitationのDelisa(第5版)とBraddom(第4版)が最近出版されたのですが、どちらも栄養の記載はほとんどありませんでした。サルコペニアの記載も1ページ弱で、欧米のリハ医学では栄養への関心が乏しいことが感じられます。

その点、この書籍は3:Bioenergetics、4:Exercise Metabolism、23:Nutrition, Body Composition, and Performance、25:Ergogenic Aidsと運動栄養学に関連する項目が4つもあります。Appendicesも運動栄養関連の項目が多いです。

Physical Medicine and Rehabilitationよりもはるかにリハ栄養の学習には有益ですので、私は当面この書籍の栄養関連の項目を読むことを頑張りたいと思っています。

小児のビタミンD欠乏

小児のビタミンD欠乏に関する総説論文を紹介します。

Misra M, Pacaud D, Petryk A, et al: Vitamin D deficiency in children and its management: review of current knowledge and recommendations. Pediatrics 122:398-417, 2008

やや長い論文ですが、以下のHPで全文見ることができます。

小児のビタミンD欠乏では、くる病が問題となります。くる病では、O脚、X脚、肋骨の骨軟骨結合部の拡大(数珠状肋骨)、頭蓋ろう、成長障害:低身長、う歯の増加、筋肉痙攣、筋緊張低下などの症状を認めます。母乳栄養で栄養不良の場合に比較的生じやすいようです。治療はもちろんビタミンDの投与です。

この論文では小児の25ヒドロキシビタミンDの血清濃度について、高度欠乏5ng/ml以下、欠乏15ng/ml以下、不足15~20ng/ml、充足20~100ng/ml、過剰100ng/ml以上、中毒150ng/mlという基準を提案しています。

成人では25ヒドロキシビタミンDの血清濃度について、欠乏20ng/ml以下、不足21~29ng/ml、充足30ng/ml以上、もしくは欠乏10ng/ml以下、不足10~30ng/ml、充足30ng/ml以上という基準がありますが、いずれにしても確立された基準はないのが現状です。

25ヒドロキシビタミンDの血清濃度が10ng/ml以下であればビタミンDを補充すべきかと思いますが、10~20ng/mlで無症状の場合には補充しなくてもよいのではと私は感じています。欠乏症状が出ていれば別ですが。

ビタミンD補充の治療方針には賛否両論があるかもしれませんが、25ヒドロキシビタミンDの測定が保険診療に含まれていないのはいずれにせよ、本当に問題だと考えます。

Abstract
Given the recent spate of reports of vitamin D deficiency, there is a need to reexamine our understanding of natural and other sources of vitamin D, as well as mechanisms whereby vitamin D synthesis and intake can be optimized. This state of-the-art report from the Drug and Therapeutics Committee of the Lawson Wilkins Pediatric Endocrine Society was aimed to perform this task and also reviews recommendations for sun exposure and vitamin D intake and possible caveats associated with these recommendations.

第3刷増刷決定:PT・OT・STのためのリハ栄養

個人的な話で申し訳ありませんが、今日、医歯薬出版の担当者から連絡があり、「PT・OT・STのためのリハビリテーション栄養」の第3刷増刷が決まりました。第3刷増刷になるとは思っていませんでしたので嬉しい驚きです。これも皆様のおかげです。どうもありがとうございました。

「PT・OT・STのためのリハビリテーション栄養」を出版させていただき1年強が経過しました。この間に「リハビリテーション栄養ハンドブック」も出版させていただいたり、PT・OT・ST・DHのNST専門療法士とメーリングリストなどで交流できるようになったり、リハ栄養の講演をさせていただいたり、私のまわりではかなり変化があったと感じています。

しかし世間的には、リハビリテーションにおける栄養への関心、栄養におけるリハビリテーションへの関心、いずれもまだまだ低いのが現状です。1年前よりは改善傾向にはありますが、PT・OT・ST・DHのNST専門療法士の人数が100倍(1100人)くらいにならないと、大きな流れ(日本のすべてのリハ現場で、バイタルサインとして栄養を評価して多職種で栄養介入してリハと栄養の効果をより高める)にはならないと考えています。

PT・OT・ST・DHで栄養に関心のある方にはぜひNST専門療法士を目指してほしいと思います。PT・OT・ST・DHのNST専門療法士メーリングリストなどで志望者のサポートをできるだけしたいと考えています。今後もリハ栄養の考え方が臨床現場に普及することに貢献したいと考えています。皆様のご支援ご協力の程、何卒よろしくお願い申し上げます。
